We accept a wide range of national and regional insurance plans to help reduce your out-of-pocket costs. Your portion of the payment, including any copay or deductible, is due at the time of your visit. If you have any questions, give us a call! We’re happy to help you understand your coverage.

Insurance & Payment Policies
  • If you plan to use insurance, a referral from your physician is required to schedule any Diabetes Self-Management Education (DSME) or Medical Nutrition Therapy (MNT) appointments. Additional documentation may be requested depending on your medical condition.
  • Specific copay and deductible rate estimates will be available once our billing department verifies benefits with your insurance carrier. It is the patient’s responsibility to provide accurate insurance information.
  • For self-pay appointments, no referral is needed. To learn more about self-pay rates and payment options, please contact our billing specialist, Walter Nash.
  • All payments are due at the time of your visit. We accept cash, check, and credit/debit card payments.
